Cooper Advanced Care Center is a uniquely designed program that is home to health care teams across a range of medical and surgical specialties.
The teams work together to provide patients with outstanding care and improved appointment availability. To achieve these standards, the team offers the following innovative models of care:
- Group visits: Group visits are coordinated so that patients with similar health conditions come together for a medical appointment and educational discussion. While we call this a group visit, each patient receives individual attention for the medical portion of their visit.
- Care coordination: Provide hospital visits and access to follow-up care when necessary in cases where patients require hospitalization.
- Same-day or next-day appointments: Our primary care practice makes every effort to schedule same-day or next-day appointments to help you manage health issues as they arise.
